How to Become a Patient

Complete the New Patient Intake Form online. It takes about 10–15 minutes and helps us understand your health history before your visit.

Bring your insurance card, photo ID, and a list of your current medications to your appointment.
Insurance
We accept most major insurance plans. Please call before your first visit to confirm we accept your plan. Our staff can answer questions about coverage, referrals, and billing.

What to Expect at Your First Visit
Please arrive 15 minutes before your scheduled time to complete any remaining paperwork. Your first visit includes a review of your medical history, a discussion of your health goals, and a physical assessment as needed. Dr. Wahidi believes every patient deserves unhurried, attentive care.
