Who May Qualify
- You're 18 or older with Medicare drug coverage (a Part D plan or a Medicare Advantage plan with drug coverage)
- BMI of 35 or higher, or
- BMI of 30–34.9 plus a condition such as:
- Uncontrolled high blood pressure
- Prediabetes
- Chronic kidney disease (stage 3a+)
- Diastolic heart failure
- Prior heart attack, stroke, or PAD
- BMI of 27–29.9 plus prediabetes, a prior heart attack or stroke, or peripheral artery disease

Good to Know
- The $50 copay is the same for everyone — income doesn't change it
- If your Part D plan already covers your GLP-1 (for example, for type 2 diabetes), you'll keep getting it that way instead
- Your doctor will confirm you're using the medication along with a diet and exercise plan
